BURRILLVILLE, R.I. (WPRI) — Police have identified the victim of a deadly construction accident in Burrillville that occurred Saturday morning.
Emergency crews responded to Victory Highway, where they found James Haczynski, 52, pinned beneath a lull forklift, a large piece of construction equipment used to move heavy materials.
Witnesses told police that Haczynski was guiding a large tree as another worker cut it down. The tree shifted, causing the machine to overturn and throwing Haczynski from it, leaving him pinned underneath.
Haczynski was pronounced dead at the scene, according to police.
The Occupational Safety and Health Administration (OSHA) and the Rhode Island Medical Examiner’s Office are investigating the incident.
